Mote College Intern Program Application Form

IMPORTANT NOTICE: THIS IS AN APPLICATION FOR THE COLLEGE INTERNSHIP PROGRAM, NOT FOR THE MOTE REU PROGRAM. REU INFORMATION CAN BE ACCESSED AT www.mote.org/reu.

Please read these instructions carefully and complete this application form in its entirety. In an effort to reduce paper consumption and our impact on the environment, emailed submissions of the application materials are encouraged. The required materials are listed below:
    1. A one- to two-page Statement of Interest that describes: a) your interests in science and career goals, b) your reasons for applying for a Mote internship, and c) why you should be selected for a Mote internship.  
    2. A copy of your current college transcript that lists all college-level courses you have taken to-date and the grades you have received. Both unofficial and official copies are accepted (if your school's grading scale differs from the 4.0 scale, please include an explanation of that scale)
    3. One (1) letter of recommendation from an individual capable of commenting on your academic qualifications and, if possible, research skills (college or university faculty or research advisors preferred).  Letters of recommendation MUST be sent from the person writing the letter, preferably as an email attachment, and will not be accepted if sent from the applicant's email.
    4. A resume or curriculum vitae describing any science-related jobs, internships, volunteer work, scholarships and/or other activities that you have held or participated in and other skills or accomplishments that you wish to identify.
    5. If you are applying for a Mote scholarship, please include the following components in your application:
  1. Check "yes" that you are applying for a scholarship.
  2. Within your statement of interest, include a paragraph that discusses your level of financial need and why you are a good candidate for a scholarship award.
  3. Submit a second letter of recommendation.
  4. Submit a copy of your Student Aid Report (SAR) from your FAFSA application.

Important reminder: Scholarship applicants must submit two (2) Letters of Recommendation, a recent copy of their FAFSA, and discuss their level of financial need and reasons why they are a good candidate for a Mote scholarship in their Statement of Interest.
