Please read these instructions carefully and complete this application form in its entirety. In an effort to reduce paper consumption and our impact on the environment, we encourage you to PLEASE EMAIL your submissions of the application materials. The list of required materials will be sent to you via email after you submit this application, and they are also listed below: |

1. A one- to two-page Statement of Interest that describes: a) your interests in the program(s) you choose and your career goals, b) your reasons for applying for a Mote internship, and c) why you should be selected for a Mote internship. (This can be sent as a Word document or a pdf file or, as a last resort, faxed or mailed to us.) |
|
2. An unofficial copy of your most recent college transcript that lists all college-level courses you have taken to-date and the grades you have received. Both unofficial and official copies are accepted. If your school's grading scale differs from the 4.0 scale, please include an explanation of that scale. (Sent as a pdf file, saved from your school's website as a jpg or png image or an htm file, copy-and-pasted into a Word document or, as a last resort, faxed or mailed to us.) |

3. One (1) letter of recommendation from an individual capable of commenting on your academic qualifications and, if possible, research skills. College or university faculty or research advisors preferred. Letters of recommendation MUST be sent directly from the person writing the letter, preferably via email as an attachment or in the body of the email; letters will NOT be accepted if sent from the applicant's email. (As a last resort, letters can be faxed or mailed but then must be on school/organization letterhead and signed.) |

4. A resume or curriculum vitae describing any relevant jobs, internships, volunteer work, scholarships and/or other activities that you have held or participated in and other skills or accomplishments that you wish to identify. (This can be sent as a Word document or pdf file or, as a last resort, faxed or mailed to us.) |

5. If you are applying for a Mote scholarship, please include the following components in your application:
1. Check "yes" that you are applying for a scholarship.
2. Have a second letter of recommendation submitted on your behalf.
3. Submit an additional document that states your level of financial need. When writing about your financial need, think about your budget and discuss as thoroughly as possible any financial difficulty you may be facing. |

It is the student's responsibility to assure that all materials have arrived on or before the deadline. Please email materials to intern@mote.org in the suggested formats above with your last name in the title (example: Lastname_Firstname transcript).

Scholarships are a one-time award of up to $2500 and are based on financial need. *Applicants: by selecting "yes," you are confirming that you are a U.S. citizen or Permanent Resident with a U.S. social security number, which is REQUIRED in order to qualify for a scholarship. You are also acknowledging that you understand you must submit additional materials, listed at the top of this application and in the confirmation email you will receive.
